It's not that common, but happens often enough that people have written up procedures for how to do it. For example, see <http://wiki.debian.org/TimeZoneChanges> and look for "change the file adding the new lines".

Yes and no -- nearly every tz release reuses names for different behavior, because releases typically update database entries.
